    Instructions for filling out the PDF Acrobat files:

These Forms are interactive and you can type in the areas on screen that you would normally write in after printing them out. Simply click in the area where information is required and type. If your mouse is over one of these hot spots you will see a balloon with the information that is requested for that item. After you have typed in all the information you can either save the file and email it back or print it and fax it. Some of these hot spots are name of show, station, market, time and date of airing, etc.
